NHS Treatment
If you would like to visit us a NHS Patient then this is our procedure:
1. We require a referral letter from your General Practitioner. This will summarise your history and append related letters and state that you are being referred as a NHS patient.
2. Once we receive your referral letter, we will apply on your behalf to your local Primary Care Trust (PCT) for the funding to cover your consultation and weight bearing X-rays. It is likely to take up to 8 weeks for us to receive a reply from them.
3. Once the reply from the PCT has been received, we will contact you. If your funding has been approved, we will arrange a consultation and arrangements to carry out our special spinal X-rays.
4. We will send you specific questionnaires referring to the history of your complaint and the way in which your life and functions are limited. These you will complete and bring with you to the consultation.
5. (If funding has not been made available then you can still attend as a Private Patient).
6. Please complete our Patient Details Form and either email it to chaygarth@spinal-foundation.org or print it out and bring it with you to your consultation.
