Getting Referred To Us
Referral Guidelines
Having looked at our Contact Us page and made an appointment, we advocate that patients attend, bringing with you:
- With a letter of referral from their General Practitioner or Specialist
- A complete "Patient Details" form
- A completed lumbar or cervical Spinal Foundation questionnaire assessing your condition, downloaded from the "Download Documents" link on the right. Please do not post the form to us, but bring them with you to the consultation
- Dynamic weight bearing X-rays either performed locally or near to the Spinal Foundation
- With any of their MRI or CT scans
Private patients please note that we do not accept payment by credit card.
